The NSCF–CTA™ Framework is grounded in a biopsychosocial understanding of wellbeing: the recognition that physical health, psychological processes and the wider conditions of a person’s life continually influence one another.
Rather than separating symptoms into purely “medical” or “emotional” causes, the framework considers how biology, history, relationships, work, identity, stress and available support may combine to shape a person’s current capacity, protective responses and sense of agency.
It brings this complexity into focus through three interacting pathways: bottom up, top down and outside-in. Together, these pathways help map how biological, psychological and social-contextual influences may be affecting capacity, threat, agency and neuro-somatic flexibility.

The Neuro-somatic case formulation-capacity-threat-agency framework. The NSCF–CTA™ Framework helps make sense of how body-based signals, thought patterns and life circumstances can interact to affect wellbeing, flexibility and functioning.
It is guided by one central principle:
When load exceeds available capacity, flexibility can narrow, protection can activate and agency can reduce.
The framework helps identify whether the next step is deeper therapeutic work, or whether the priority is first to reduce load, strengthen safety and restore capacity.

The three interacting directions of influence. The BU / TD / OI model maps where pressure may be coming from.
Body-Up includes sleep, pain, fatigue, illness, hormones, sensory load and physical capacity.
Top-Down includes thoughts, expectations, memory, self-pressure and threat prediction.
Outside-In includes work, relationships, caregiving, culture, finances and practical demands.
Rather than choosing between body, mind or environment, formulation considers how these influences may be interacting.

Understanding what the system is carrying. The framework considers three key questions.
Capacity: What resources are currently available to meet demand?
Threat: What is the system having to manage, predict or brace for?
Agency: Where is there choice, influence or room to respond?
This helps identify where pressure is exceeding available resources, where protection may be activating, and where practical or therapeutic support may help restore options.

The ability to adapt without being stuck.
Neuro-somatic capacity flexibility describes how easily a person can move between focus and rest, activation and recovery, connection and self-protection, or coping and adapting.
The aim is not constant calm or perfect regulation.
It is to create more options when life becomes demanding, so the system is less likely to remain stuck in overdrive, shutdown, depletion or rigid coping patterns.
